How do I Request a Prescription?

If your doctor has authorised repeat prescriptions you may request them in one of the following ways:

• On-Line, by clicking here and completing the electronic form. Please be aware that transmitting this confidential information in this way is not secure, and that by using this form you accept the risk that the information you send could be obtained by an unauthorised party

• By Phone, by calling 0844 477 3603.

• By Fax, by faxing 0844 477 3683.

Please give 2 working days notice (excluding weekends) before collection of repeat prescriptions.

If you have an arrangement for a local chemist to receive your prescription, you will now need to contact the Chemist to inform them that you have ordered your repeat prescription and request that they collect it.
This is a change from the existing scheme - so please make sure you inform your chemist each time you order and they will arrange collection.

If you are not ordinarily resident in the UK (regardless of tax, national insurance, pension or UK address) and requesting medication, please be aware that it is not possible for the practice to supply this to you. Please see "Non UK Residents" for more details.
